We need to have a copy of your state's tax and use permit and a copy of a completed Form 01-339 on file. We will be unable to activate your account if you send only one of these forms to us, and it will cause a delay in activating your account or cause us to delete your wholesale account. We will need to have both of these forms on file.
We have pasted several links at the bottom of this page for you to click on to get the Form 01-339. Please click on the link that is best for you, the forms comes with a .doc file which is used by Mircosoft Word or we have .pdf which will be opend by Adobe Acrobat Reader. Please fill out with your states information in the blanks. When you have completed this form please fax, email or snail mail Form 01-339 and a copy of your state's tax permit.

Note: If from any country other then the United States please call us or email us concerning any questions that you may have on setting up an wholesale account. Tax numbers come in differnt forms or called differnent names example of these are as follows: In the United Kingdom, England they us the word VAT number or in Australia they use Australian Business Number(ABN) these are just a few examples. when filling out these forms please use your Country's information in the blanks on the Form01-339.
